The controller of your personal data is Eye Centre Prague, a.s., Company ID 26169231, with its registered office at Holešovice 1637, Prague 7, 170 00 (hereinafter referred to as the 'Controller'), in accordance with the Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (hereinafter referred to as the 'Regulation').
WHAT DATA DO WE PROCESS AND FOR WHAT PURPOSE
1) Website visit - cookies:
When visiting the website, cookies are processed. Some of these files allow the website to function properly and connect your activities while browsing the site.
The collection of cookies for this purpose may be considered personal data processing. Your objection will then be evaluated promptly, no later than 30 days from receipt. Conversion Measurement using Google Ads
On our website, we use conversion measurement through Google Ads, which includes the functionality of enhanced conversions to increase the accuracy of measured conversions. Google may match this data with its database for the purpose of refining conversion measurement. 2) Sending the contact form or appointment request
When using the contact form, the following personal data is processed:
- Your first name, last name;
- Your email address;
- Your phone number;
- Your age;
- Your address;
- IP address;
- Health issues you describe.
The processing of personal data – first name, last name, phone number, and email address is allowed under Article 6(1)(b) of the Regulation – processing is necessary for the performance of measures taken prior to the conclusion of a contract at the request of the data subject. This personal data must be processed to enable communication between you and the Controllers.
In the case of processing personal data concerning the health issues you describe, this is considered the processing of a so-called special category of personal data. This processing is necessary for the purpose of recommending suitable services offered by the Controllers, but it is entirely voluntary. However, your explicit CONSENT is required for such processing, which you grant by clicking on "acknowledgment/consent to the processing of personal data." Such processing is then permitted under Article 9(2)(a) of the Regulation.
The Controllers process personal data in the following manner:
- A response to your inquiry will be sent to your email.
- The IP address is stored to recognize any spamming activities and to defend against such activities by blocking access for those IP addresses. This processing is permitted under Article 6(1)(f) of the Regulation – legitimate interest of the Controllers. You have the right to object to this processing of your IP address at recepce@ocp.cz.
This data will be processed by the Controllers for the duration of discussions regarding the service, but no longer than 3 years from the last communication if no contract is concluded. In such a case, communication will be archived for 10 years from the last part of the service execution for the purpose of potential legal defense of the Controllers and due to applicable legislation in the field of healthcare services.
3) Sending an inquiry to the online consultation
When submitting an inquiry to the online consultation or when providing a reference, we process the following data:
- Name or nickname
- Email address
- IP address
- Any additional personal data that you voluntarily include in the text of your contribution
The processing of personal data is permitted under Article 6(1)(a) of the Regulation – your consent. In the case of processing personal data concerning the health issues you may describe, this is considered the processing of a so-called special category of personal data. Providing this data is entirely voluntary. However, your explicit CONSENT is required for such processing, which you grant by clicking on "acknowledgment/consent to the processing of personal data." Such processing is then permitted under Article 9(2)(a) of the Regulation.
All of this personal data will be processed by the Controllers for a period of 6 years from the publication of your inquiry or the provision of a reference.
The IP address is stored to recognize any spamming activities and to defend against such activities by blocking access for those IP addresses. This processing is permitted under Article 6(1)(f) of the Regulation – legitimate interest of the Controllers. You have the right to object to this processing of your IP address at recepce@ocp.cz.
4) Subscription to the newsletter or downloading a guide
When subscribing to the newsletter or downloading a PDF guide, we process the following data:
- Email address
- IP address
We will process your email address for a period of 3 years from the granting of your consent for the purposes of sending commercial communications. Such processing is permitted under Article 6(1)(a) of the Regulation. You can withdraw your consent at any time at recepce@ocp.cz or by unsubscribing from the subscription in the commercial message sent to you.
If you have ordered a guide for download, we will process your email address for the period necessary to fulfill your order, but no longer than 3 years. Such processing is permitted under Article 6(1)(b) of the Regulation.
The IP address is stored to recognize any spamming activities and to defend against such activities by blocking access for those IP addresses. This processing is permitted under Article 6(1)(f) of the Regulation – legitimate interest of the Controllers. You have the right to object to this processing of your IP address at recepce@ocp.cz.
5) Questionnaire "Rate us"
When filling out and submitting the "Rate Our Work" questionnaire, we process the following personal data:
- First name
- Last name
- Any additional personal data that you voluntarily fill in the questionnaire or send as an attachment
The processing of personal data is permitted under Article 6(1)(a) of the Regulation – your consent. In the case of processing personal data concerning the health issues you may describe, this is considered the processing of a so-called special category of personal data. WHO PROCESSES THE DATA
The processing of personal data is carried out by the Controller, but these processors may also process personal data on behalf of the Controller:

- INDIGO Multimedia, s.r.o. (website, web hosting, and CRM system provider)
- Atweb Consulting, s.r.o. (marketing agency)
